To ensure a smooth and efficient intake process, please gather the following information to provide to our office:
Initial Documents:
- Prior year’s tax return (unless prepared by our office)
- Social Security numbers and birthdates for children and other dependents
- Client intake form for the current tax year
Income Documentation:
- All W-2 forms from employers
- Statements for miscellaneous income (odd jobs, contracts, etc.)
- Bank-issued interest statements (Form 1099-INT)
- Investment company dividend statements (Form 1099-DIV)
- Unemployment insurance statements (Form 1099-G)
- State tax refund statements (if applicable)
- Schedule K-1 forms from partnerships, estates, or S-corporations
- Gambling winnings and losses
- Alimony received or paid
- Social Security statements
- Stock/bond transaction statements (Form 1099-B) and purchase details
Small Business or Rental Property Records:
- Prior year’s business return (unless prepared by our office)
- Categorized income and expense statement (profit & loss)
- Business asset list, including depreciation schedules
- Business mileage log
- Business-use percentages for shared assets (e.g., 75% for business)
- Subcontractor details and amounts paid
- Health insurance premium payments
- Access to QuickBooks Account
- Home office details: square footage, utility expenses, insurance, repairs, improvements, and purchase information
Deductions:
- Mortgage interest statement (Form 1098)
- Real estate taxes paid
- Mortgage points paid on a new home or refinance
- State taxes paid in the previous year (if applicable)
- Irrigation tax (if no water access)
- Vehicle taxes and registration fees

- Classroom supply expenses (for teachers only)

Investment Expenses:
- Safe deposit box fees, IRA management fees, investment advice, investment-related publications, and education
Additional Items:
- Form 1099-SA for HSA Distributions
- Form 1095-A if health insurance was provided through the marketplace
- Form 1099-Q for distributions from 529 Plans
- Closing documents for real estate transactions (buying, selling, or refinancing)
- Child care provider information (name, address, Tax Identification Number) and amount paid
